Notice of Privacy Practices

Superior Insurance Agency of Sault Ste. Marie, LLC (the Agency) and its Agents are authorized by State and Federal Law to request and obtain Personally Identifiable Information (PII) to obtain proposals and/or enroll at our clients’ request, in a life plan, health plan, or other related product of the clients’ choosing, on or off the Federally Facilitated Marketplace, as well as the related servicing of these plans. The Agency and Agents may also obtain Personal Health Information (PHI) to assist clients in choosing the most appropriate life and/or health plan(s) for their needs as well as to service plans once they are in place.

The Agency and its Agents have legal authority to request and maintain PII and PHI, provided voluntarily by clients. Clients understand that by Federal Law, if they choose not to disclose certain PII, they may not be able to apply for Health Insurance and related coverage. Information will not be shared with other entities except for the purposes stated above or used for any other purpose without express consent from the client

Handling of PII Breach/Incident

Per Centers for Medicare and Medicaid Services (CMS), any incident or potential incident of mishandling or unauthorized access of Personal Identifying Information (PII) will be reported to the CMS IT Service Desk. Incidents include but are not limited to information being sent to an incorrect recipient via fax, mail, or email; loss or theft of data storage devices; access of electronic or paper document storage containing PII by unauthorized person(s) or entities. Incidents and potential incidents will be immediately reported upon discovery by calling 1-800-562-1963 or emailing cms_it_service_desk@cms.hhs.gov.

Security

This site has security measures in place to protect the loss, misuse and alteration of the information under our control. All data is protected using the most advanced methods available. We do not store financial information like credit card numbers or personal information like social security numbers on this site.
